Fragmented care is a major problem in the U.S. healthcare system. Fragmented readmissions occur when a patient is readmitted to a different hospital than the one they were initially discharged from. Care fragmentation occurs in one quarter of readmissions and is associated with worse health outcomes. Led by Dr. Sara Turbow, Research@EmoryGIM seeks to identify solutions to reduce care fragmentation and its impact on health outcomes. Other research focuses on prescribing practices and provider burnout.
