Why

To honor the dedication and achievements of those serving others through their work in primary care. 

 

Who

  • Actively engaged (≥ .40 effort) in longitudinal, comprehensive primary care within Emory Clinic, Emory Specialty Associates, or an Emory affiliate (VA, Grady, Emory University Student Health, Emory Corporate Medicine, Emory linked senior care sites) as a staff member or provider.
  • Provider disciplines include geriatrics, family medicine, internal medicine, pediatrics, nurse practice, physician assistant, preventive medicine, and palliative care.
  • All ranks of faculty

What

You do not have to already be an award recipient within your section, unit, or division. Yet, if you have already been recognized elsewhere, you are still eligible for a Primary Care Consortium award.

PCC Award Categories

Awards for Practice Sites/Team

  1. Achievement in Primary Care Innovation: The PC team that has developed and implemented breakthrough solutions to today's problems
  2. PC Team-Spirit Award for Excellence in Clinical and Staff Wellness: The PC team that has created a workplace and support structure that has improved a strong culture of wellbeing

Open Category Awards

 An individual of any discipline or a unit or team may be nominated:

  1. Transformational Primary Care Leadership: Steady and supportive leadership guiding us all through difficult times
  2. Primary Care Career Achievement Award (≥ 25 years): Sustained contributions to primary care throughout a 25 year or more career.
  3. Primary Care Diversity, Equity, and Inclusion Champion: Demonstrated commitment and achievement in championing DEI principles in primary care
  4. Rising Star Recognition in Primary Care (≤ 5 years): Outstanding early-career achievement in patient care, leadership, and/or academics
  5. Volunteerism in Primary Care- Community Connection Award: An individual or unit within PC that contributes in a manner that enhances the health and wellbeing of the community

Awards for Individual Nursing and Administrative Staff

  1. Award for Primary Care Outstanding Business or Clinical Administrator: Exceptional achievement over the past year.
  2. Award for Primary Care Outstanding Clinical Support Staff: Exceptional achievement over the past year.
  3. Problem Solved – Value-Added Award: The individual who this year solved the problem that drove everyone crazy and also brings value to the organization every day.

Awards for Individual Providers

  1. Achievement in Primary Care Clinical Research - Yearly Contribution: Outstanding achievement over the year in Primary Care research
  2. Achievement in Education in the Primary Care Setting: Outstanding achievement over the year in Primary Care education
  3. Outstanding Primary Care Leadership Award- Site Lead: Exceptional leadership in the role as provider site lead for primary care
  4. Outstanding APP Primary Care Provider: Exceptional year performance as an APP in primary care
  5. Outstanding Physician Primary Care Provider: Exceptional year performance as a primary care physician
